EOP Transfer Student Services Agreement Plan
This agreement is designed to outline the EOP requirements. Please review.
General Requirements
I agree to meet with my EOP counselor once per year ( Fall semester )
In order to remain eligible for the EOP grant, I agree to maintain full-time enrollment (12 units) per semester.
Note: Due to budget limitations, not all EOP transfer students will receive the EOP grant. The Financial Aid
office will determine eligibility and award.
I agree to enroll in Ethnic Studies 198 (1 course, 1 unit). A one unit course designed to aid in the transition to the
UniversityMandatory. Detailed information will be sent to you on this course prior to enrollment.
EOP Learning Community- (3 class sequence) participants will be awarded a one-time stipend of $ 200.00.
Financial Aid office will determine eligibility and award the funds. This will be disbursed mid-September. This
is a one-time disbursement and is based on financial need. – Optional
If my GPA should fall below 2.00, or if I am placed on Academic Probation or Academic Disqualification, I
agree to meet with my EOP counselor for advising/academic planning
I agree to seek tutorial assistance in the PARC Center or major department, as needed or referred
I agree to declare my major upon completing 60 units ( University requirement )
I agree to participate in the EOP retention (ARISE) services and utilize program resources as outlined by my EOP counselor.
Junior thru Senior Requirements
I agree to participate in the EOP retention services (ARISE) :
o Completion of my General Education Plan
o Career Workshops
o To attend a WPJ preparation workshop (if needed)
o Assistance in applying for graduation and graduate school
o Attendance at Student Success workshops (resume writing, personal statements, cover letters, etc.)
University Advising Policy
I understand that University policy requires me to seek and meet with an advisor in my major academic department in
addition to general EOP counseling/advising.
